Video Summary
The virtual AANN symposium featured presentations on critical-care priorities for traumatic brain injury (TBI) and spinal cord injury (SCI).

Michelle Vandermeer used a severe TBI case to explain how injuries can worsen after the initial impact. She emphasized preventing secondary brain injury through rapid airway and oxygenation support, avoiding hypotension and unnecessary hyperventilation, frequent neurological and pupil assessments, temperature and glucose control, seizure management, and appropriate coagulation reversal. She also reviewed ICP monitoring, osmotherapy, escalation for refractory pressure, and the importance of beginning rehabilitation early.

Ellen Harvey discussed SCI assessment and ICU care, highlighting the need to prevent further cord injury and complications. Key priorities included spinal motion precautions, detailed motor and sensory examinations, respiratory support, maintaining cord perfusion, managing bradycardia and neurogenic shock, preventing infection and blood clots, and addressing pain, dysautonomia, skin injury, and bowel and bladder dysfunction. She stressed that psychological support, patient independence, and coordinated rehabilitation are essential parts of care.

The speakers and attendees also discussed seizure treatment, temperature monitoring, bladder-management protocols, and challenges in coordinating spinal precautions. Both presentations underscored the central role of nurses in recognizing deterioration, preventing secondary injury, advocating for patients, and coordinating multidisciplinary care.
