Pdf Summary
Malignant middle cerebral artery (MMCA) stroke is a large ischemic stroke that can cause rapidly progressive cerebral edema, brain herniation, severe disability, and death. Deterioration may occur within hours and is most likely during the first several days. No single test reliably predicts which patients will worsen, so clinicians combine neurological findings with imaging. Reduced consciousness, pupil changes, and declining motor responses are important warning signs; CT and MRI can help estimate infarct size and edema risk.

Care requires a coordinated neuroscience intensive-care team. Management includes frequent neurological assessments, airway and aspiration precautions, and careful control of blood pressure, glucose, and temperature. Sedatives should be minimized when possible to preserve the neurological examination. Hyperosmolar agents such as mannitol or hypertonic saline may be used as rescue treatment or a bridge to surgery, but evidence for routine preventive use is limited. Early endovascular treatment may reduce edema risk in some patients.

Decompressive hemicraniectomy (DHC) is a potentially lifesaving intervention, particularly when performed early, but survival does not guarantee good functional recovery. Outcomes and the likelihood of substantial dependence should be discussed with the patient or surrogate when considering surgery; age and the patient’s values and preferences are important. After surgery, care includes continued neurological and hemodynamic monitoring and precautions to protect the surgical site.

Nurses play a central role in detecting deterioration, communicating changes promptly, coordinating care, and supporting patients and families through uncertain, high-stakes decisions. Clear communication, family meetings, and discussions of goals of care help align treatment with the patient’s wishes.
