Video Summary
The lecture reviews a practical approach to migraine assessment and management. A detailed history should establish headache location, quality, frequency, duration, associated symptoms, triggers, medication use, medical history, lifestyle habits, and the patient’s goals. A neurologic examination—including fundoscopy—is important, and red flags such as sudden onset, new headache after age 50, progressive or positional pain, systemic symptoms, papilledema, or abnormal neurologic findings may warrant further investigation.

Migraine commonly features throbbing pain, nausea, and sensitivity to light and sound; aura symptoms are reversible and typically occur shortly before the headache. Medication-overuse headache is common: frequent use of acute pain medicines can perpetuate headaches, and stopping the overused medication is essential.

Treatment has two components: acute medicines to relieve attacks and preventive treatments when headaches are frequent or disabling. Options include triptans, CGRP-targeting medicines, and established preventives such as amitriptyline, beta blockers, and topiramate. Selection should account for medical conditions, side effects, pregnancy considerations, patient preferences, cost, and insurance requirements. CGRP therapies can be effective, but may require prior treatment trials and several months to assess benefit. Botox is generally reserved for chronic migraine.

The speaker emphasizes limiting acute medication use, addressing sleep, hydration, meals, exercise, stress, and caffeine, and considering supplements or non-drug therapies. Two case examples show how individualized treatment—combining prevention, acute medication, and lifestyle changes—can substantially reduce headache frequency and disability.
